You can start a screw with an awl or other pointed tool. Make a small hole insert the screw and turn to the right with a Phillip's screwdriver.

The screws are small but 1/4" leaves nothing to screw into. Use super glue or Gorilla glue. They will bond metal to wood of plastic.
That depends on the weight of what you are hanging. Any fine wire will do for most applications. In doubt use multiple strands of wire.

It's about 3/4" or so. As you attach these in pairs to the back of the frame about 3/4 of the way up, then attach picture hanging wire betwen them. These are not meant to be at the top center as it can cause your pictures to hang crooked.

No they don't. Just the same size screw to hold the hanger onto whatever it's attached to.
Here's a trick. Fill the hole with a sliver of wood soaked in glue. Wait for it to dry, sand smooth. Put screw into old spot now filled with something for the screw to hold on to.

One-size screw. Hole large enough for that. If your holes have been stripped, insert a drop of wood glue and stuff in a toothpick or two and trim it off when dried. You can then use the included screws.
You may have to drill a larger hole than exists. The screw holes only fit a #6 (I think) screw.
