This is a great example of what might be called a "manufacturing set." The cost of each part is the same and the entire set ... so buy as many sets as you need and simple put the extras in a draw in case you need the later.
A template was not included in the kit I received. The mounting bracket has adjustments for up/down and side to side as the holes are slotted, this gives you some wiggle room.

There was no template included, what I did was measure the spacing of the holes while off the wall and measured carefully.
I agree this would be helpful from the manufacturer. However, I’ll tell you how I resolved this. I use painters tape and and place on the back of the bar mounts stretched across between each of them. Then I mark the holes. I then remove the tape and place it on the wall using a level to make sure the holes are levels. Viola, you just created your own template. Hope this helps.

If it doesn't come with the kit the you might get off the web site otherwise no.
The mounting plate can be removed from the individual pieces and used as a template to mark holes on the wall.
There was no template in my pack but it is easy enough to do by simple measurement. The way the brackets fit the bar, there is a bit of room for error/tolerance.

I used an anchor so it holds my robe quite easily. If you install it into stud ,even better.
it depends on two things, how it is on the wall, if its on the drywall then a few pounds to hang a robe or a towel, if it is on the stud then maybe a bit more weight but regardless a few pounds.
